Applicant Information Request

The Applicant Information Request form is sent by the hiring unit to each applicant who applies for a position that is:

  • Unclassified
  • Temporary or Permanent
  • Full-time or Part-time

All information on this form is to be provided voluntarily by the applicant.

The purpose of the Applicant Information Form is to provide information on the applicant's race, sex, and veteran status. It is entirely legal and proper to make efforts to identify the race and sex of applicants, and to consider these factors in identifying the most suitable person for a position. The hiring department should complete the top section of the form before forwarding it to the applicant.  The form should be sent as soon as possible after receiving the application to provide ample time for the applicant to voluntarily respond. After the form is completed by the applicant, they should return it to the Office of Human Resources either via email or by using the mailing address provided on the form. Departments should instruct applicants responding via email that they may type in their names as opposed to providing a signature on the form.

It is illegal to discount someone because of a their race or sex. In making the final hiring selection, however, it must be noted that the best suited applicants for a vacancy are those not only meeting the position's requirements, but those meeting the affirmative action needs of the job group as well. It is critical that the department administrator and the associated college community and diversity coordinator communicate the established goals for affirmative action efforts when recruiting for each position so that the search committee has proper direction in developing selection criteria.
