All interviews should be carefully planned and conducted in accordance with affirmative action and equal employment opportunity guidelines. The Record of Interview form is used to document the strengths and weaknesses of applicants interviewed for all advertised positions. A separate form should be used for each individual interviewed. The information entered on this form should support the decision to select the final candidate. While these forms do not need to be submitted for approval to the Office of Access & Equity, they must be readily available should there be a question concerning the selection process. Search committees for unclassified searches may choose to use their own means of documenting their evaluation of each candidate. Regardless, these documents or forms are to be maintained as part of the department's recruitment file.
