Supervisor Evaluation Form
Used to evaluate your intern’s overall performance, position performance, and work habits. This form needs to be filled out and returned to the department near the end of the student’s internship.
Intern Employer Day Registration
Register for the upcoming Intern Employer Day either online or by printing out the registration form and sending it to Linda Kanaley by mail, e-mail, or fax.
