Equipment Check-out and Return

In some cases, equipment may be checked out to perform work duties off-site. The process to check out equipment is as follows:

  1. Download form
  2. Complete contact info, length of time and purpose
  3. Record proper equipment information (Leave the "Modified" section blank. It is for Business Services use only.)
  4. Obtain employee signature and manager signature
  5. Send completed form to College Station through mail system

*Note: Purpose must be completed. Also, the form must be signed by the manager.

To return equipment, the manager must send an email to colstat@clemson.edu with a simple description of the equipment borrowed and when it was returned.
