for Ending TERI

Ending PRIOR
to full TERI service: STEP 1 Please
provide a letter of intent stating full legal name, TERI end date, original
signature, department, and Social Security Number. Please mail letter of intent to: Office of Human Resources Retirement Services STEP 2 You will be contacted to schedule an appointment to complete SCRS Form 7202 Pension Withholding Certificate/ Automatic Deposit Authorization SCRS Form 7500 TERI Payout Election Duplicate forms can be accessed at http://www.retirement.sc.gov/forms/default.htm. You must attach a legible copy of your
driver's license or special identification card issued by your State
Department of Transportation or Public Safety, and return along with the
forms to: |
Completing
your full 60 month TERI service: STEP 1 Approximately
90 days prior to your TERI end date you will receive
from the South Carolina Retirement System a
TERI payout package that provides TERI termination date, rollover and direct
payment options, basic tax liability, and/or early withdrawal penalty
information. STEP 2 Please
provide a letter of intent stating full legal name, TERI end date, original
signature, department, and Social Security Number. Please mail letter of intent to: Office of Human Resources Retirement Services STEP 3 You will need to complete the following forms enclosed in the

RETIREE INSURANCE
ENROLLMENT: Upon receipt of the letter of intent for TERI
participants ending TERI prior to the 60 month TERI service, retirement staff
will notify the insurance unit. A benefits
counselor from the insurance unit will contact the employee to schedule an
appointment to cover options and paperwork. If a TERI participant works their
full 60 month TERI service, a benefits counselor from the insurance unit will
contact them to schedule an appointment approximately one month prior to the
TERI termination date. Paperwork must be completed within 31 days of the date employees
end TERI to enroll in retiree insurance.
Eligibility rules apply. It is
the employee’s responsibility to complete and forward all retiree insurance
paperwork within the designated deadline. ANNUAL LEAVE PAYOUT
DEFERRAL:
If employees ending TERI wish to defer an annual
leave payout, they need to call the general benefits number at (864) 656-2713
to make an appointment. It is essential employees call and make
an appointment 45 days prior to the date they will end TERI for assistance
with the deferral. Failure
to schedule an appointment within this time period, may result in not being
able to defer the annual leave money and instead being paid out the money,
which will be subject to taxes. INSURANCE

RETIREE BENEFITS: Retiree ID Cards: Please indicate your desire to have a Retiree ID card issued. This can be done as part of your ending
TERI letter, or before your effective TERI end date. The Retiree ID card will provide access to various
campus services including the library, approved athletic events, forums,
etc. Your request, along with a
certified retirement date will be forwarded to the Tiger 1 office, who will
issue the Retiree ID card. Computer Access: Please have your department
chair/head send authorization to include the following: your full name,
social security number, Clemson userid, and
department number to Patty M. Lusk (by telephone at 656-0540), or by e-mail
at crdesk@Clemson.edu or plusk@clemson.edu before your
retirement date, to arrange for your computer ID to be continued. Requests made after your retirement date
are much more involved and will require greater time and are subject to not
