Q: What should I do to get started?
A: Go to our Getting Started page.

Q: I can't download the forms… what do I do now?
A: Contact our IBC Coordinator, and she will help you. You can send her e-mail ventura@clemson.edu, or call her at 864-656-0118.

Q: I have some questions about the policies and regulations governing the use of hazardous agents at Clemson University, so who do I call?
A: You can contact the IBC Coordinator, via email ventura@clemson.edu or call 864-656-0118. You may also contact Mr. Robin Newberry, Chief Environmental Health and Safety Officer, at wnewber@clemson.edu or call 864-656-1806.

Q: How do I sign up for the occupational health and medical surveillance programs?
A: Please contact Sue Pedrick , RN, Nursing and Wellness Center at 864-656-5529.  You can also train online at http://www.hehd.clemson.edu/MSP/.

Q: My grant proposal is due next week, and I have to get approval! Help, what do I do? I can't wait for the next committee meeting!
A: Please contact the IBC Coordinator, ventura@clemson.edu for assistance at 864-656-0118.

Q: My project has changed quite a bit. What should I do now?
A: You will have to file an Amendment Form. That amendment will be reviewed by the IBC. The reviewers may then ask you to submit a new protocol depending upon how significant the changes actually are. Please remember that ALL changes should be reported to the IBC, no matter how insignificant you may think it is!

Q: Can't I wait until the annual review period to update and make significant changes to the protocol?
A: No, as that would be out of compliance with federal regulations. Please contact the IBC Coordinator for further assistance.

Q: I need to change the personnel listed on the Hazard Sheet. What do I do?
A: Please file a new MUA Hazard Acknowledgement sheet. This should list ALL current personnel associated with this study.

Q: My project is finished. Do I need to tell anyone?
A: Yes, please contact the IBC Coordinator as soon as possible.
