Disclosure & Copyright Forms

Forms are available in both Microsoft Word and PDF formats.  Select the format that is most convenient for your purposes.

Please note that Adobe Acrobat Reader 5.0 will let you save a copy of a PDF file to your local hard drive.  However, it does not allow you to save the form with completed form fields.  To save your data, you must use the full version of Adobe Acrobat.

Completed Confidential Invention Disclosure forms or Confidential Computer Software Copyright Disclosure forms may be submitted to the Office of Technology Transfer, to the attention of Janet Dillon, in one of three ways:
