Patient/Client Rights

These are the expectations that you, as a health care consumer, have a right to expect in any health care visit.

  1. Treatment without discrimination as to race, color, religion, sex, national origin, age, disability, or status as a disabled veteran and/or veteran of the Vietnam era.
  2. Care that is considerate and respectful.
  3. Participation in decisions regarding treatment.
  4. The name and function of RHC personnel providing services and the identity of other personnel and institutions assisting in treatment.
  5. Appropriate assessment and management of pain.
  6. Complete and current information concerning your diagnosis, treatment and expected outcome in terms you can be reasonably expected to understand.
  7. Receive information regarding condition/treatment options.
  8. Refuse treatment and to be informed of any potential consequences.
  9. Privacy to the extent consistent with care.
  10. Privacy of all records pertaining to your treatment, except as otherwise provided by law.
  11. Access the information contained in your records within the limits of the law.
  12. Continuity of care and to be informed of the possibility of continuing requirements following the end of treatment.
  13. Refuse to participate in research.
  14. Examine and receive an itemized statement of all charges.
  15. Upon request, full disclosure of facility's rules and regulations.
  16. A smoke free environment.
  17. An environment free from mental, physical, sexual and verbal abuse, neglect and exploitation by staff, students, and visitors.

Patient/Client Responsibilities

These are the responsibilities of a health care consumer to the provider of services.

  1. Provide accurate information on complaints, including pain, past illnesses, hospitalizations, medications and psychological treatment.
  2. Help us assess your pain and inform us if your pain is not relieved.
  3. Tell us if you don't understand instructions and/or information.
  4. Keep appointments.
  5. Follow treatment plan.
  6. Clearly state refusal of treatment.
  7. Follow Redfern Health Center rules and regulations.
  8. Consider the rights of other patient/clients and staff.
  9. Respect other's property.
  10. Refrain from smoking while in facility.
