All new and transfer students are required to:
- Complete the Online Medical Clearance Form.

(Official documentation includes those indicating immunization history, in ENGLISH, and signed by a physician, PA, PA-C, FNP, FNP-C, nurse; or an official stamp from a health care facility, World Health Organization, or Publich Health Department; Military records)
If you don't already have a verified record of your immunizations, you may print out an immunization form that your health care provider can fill out and sign.
*Students not in compliance with immunization requirements will not be allowed to complete registration for the next semester, and may be subject to a late fee.
All Students:
Two doses of MMR, (Measles, Mumps, and Rubella vaccine) after 12 months of age.

International Students or Residents:
Tuberculin (TB) Skin Test (TST)(formerly known as PPD)
Any student who has resided in the following countries identified by the U.S. Centers for Disease Control and Prevention and/or the American College Health Association, where tuberculosis is endemic, is required to be screened upon arrival to Clemson. TB screening will not be accepted from outside of the U.S.

Students with a prior positive PPD must bring a copy of a chest x-ray report, (translated in English) within the last year. Students who have a prior positive PPD or who are receiving treatment for tuberculosis should bring copies of all treatment records, including medical notes, laboratory reports, and x-rays. The cost of the tuberculin screening is nominal.
Recommended Vaccines

Tetanus/Diphtheria/Pertussis (recommended)
Booster: Tdap (preferred) (tetanus toxoid, diphtheria, and acellular pertussis) is recommended with at least five years since the last dose of Td. Administer with meningococcal conjugate (MCV4) vaccine simultaneously if both are indicated.
Booster: Td (tetanus toxoid and diphtheria) is recommended if Tdap is not available or was administered previously.

Meningococcal vaccine (highly recommended)
Meningococcal Tetravalent conjugate; administer simultaneously with Tdap if possible.Any entering student age 25 or under should carefully review the information on the meningococcal vaccine on our website and discuss potential benefits of vaccination with his/her individual healthcare provider.

Quadrivalent Human Papillomavirus Vaccine (HPV) (strongly recommended)
A series of 3 vaccines is recommended for all female college students 11 to 26 years old.

Varicella (recommended)
Either a history of chicken pox, a positive Varicella antibody, or two doses of vaccine given at least one month apart if immunized at the age of 13 or older. -
Hepatitis B (highly recommended)
A series of 3 vaccines is recommended. May also be combined with Hepatitis A.

Hepatitis A (recommended)
A series of 2 vaccines is recommended for routine use in all adolescents through the age of 18 and in particular for adolescent and adult high-risk groups (i.e., persons traveling to countries where hepatitis A is moderately or highly endemic, men who have sex with men, users of injectable and noninjectable drugs, persons who have clotting-factor disorders, persons working with nonhuman primates, and persons with chronic liver disease). May also be combined with Hepatitis B in a series of 3 vaccines.
