Submission of this form is mandatory for all new and transfer students coming to Clemson University.

• All Students:

  Two doses of Measles, Mumps, and Rubella vaccine (MMR) after your first birthday. (Not required if you were born before 1957.)

• International Students:

  Tuberculin (TB) Skin Test (TST)(formerly known as PPD): New international students arriving from countries identified by the U.S. Centers for Disease Control and Prevention as high risk for tuberculosis will be screened upon their arrival in Clemson.

To complete the online medical clearance form you will need the dates that you received your immunizations and any allergies to medications. If you do not have any record of your immunization history, you may want to check the following resources:

• Your pediatrician/family doctor

• Your public health clinic

• Your high school

• Previous colleges/universities

If you still cannot locate any documentation, you will need to obtain two MMR vaccines and then submit the online medical clearance form. You will have to wait 28 days between receiving dose one and dose two.

If you exit the form prior to final submission, your information will not be saved and you will need to start over again at the beginning. So make sure you have all of your information before you begin.

2008 Summer/Fall freshmen and graduate students: July 18, 2008

2008 Fall transfer students: August 1, 2008

2008 Fall late acceptants: Within 4 weeks of acceptance

2009 Spring freshmen, transfers, graduate students: December 12, 2008
