Completion of this online form is MANDATORY for all new and transfer students, in addition to submitting the official documentation to Redfern Health Center by the deadline noted below.

• All Students:

  Two doses of MMR, (Measles, Mumps, and Rubella vaccine) after 12 months of age.
  **Exempted if born before 1957**

• International Students or Residents:

  Tuberculin (TB) Skin Test (TST)(formerly known as PPD)

Any student who has resided in the countries identified by the U.S. Centers for Disease Control and Prevention and/or the American College Health Association, where tuberculosis is endemic, is required to be screened upon their arrival to Clemson.

To complete the online medical clearance form you will need the dates that you received your immunizations and any allergies to medications. If you do not already have a verified record of your immunizations, you may print out an immunization form that your health care provider can fill out and sign.  You may also want to check the following resources:

• Your pediatrician/family doctor

• Your public health clinic

• Your high school

• Previous colleges/universities

If you still cannot locate any documentation, you will need to obtain two MMR vaccines and then submit the online medical clearance form. You will have to wait 28 days between receiving dose one and dose two.

If you exit the form prior to final submission, your information will not be saved and you will need to start over again at the beginning. So make sure you have all of your information before you begin.

2009 Summer/Fall freshmen, transfers and graduate students: July 1, 2009
