This form is to request use of the Union Recreation Center. Requesting a reservation does not insure that you have a reservation. A Reservation Confirmation will be e-mailed to the address you supply when the reservation is reviewed accepted. It is a good practice to keep this confirmation until after the event. You may fill out a downloadable version of this form and complete the form returning it to the Union Recreation Center or fax to (864) 656-5828.
