Pre-Matriculation/Immunization Requirements

All students are required to:

1) Complete the Required Student Immunization Forms
2) Complete the Pharmacy Prescription Card Information
3) Submit this official documentation prior to the following deadlines:

Summer/Fall enrollment: July 1
Spring enrollment: December 1

If you are unable to obtain a verified immunization history, all required immunizations are available to you at Redfern Health Center. Students not in compliance with immunization requirements will not be allowed to complete registration for the next semester and might be subject to a late fee.

Exception:  If you are enrolled in only online courses, you do not need to comply with medical clearance requirements.  However, if you receive notices of non-compliance, contact Dede Leylek, Director of Administrative Services at 864-656-7292 or e-mail kleylek@clemson.edu.

Required vaccines

Required vaccines, effective January 1, 2012:

  1. MMR (measles, mumps, and rubella): for students born after 1956, two doses required after 12 months of age.
  2. Tdap (tetanus, diphtheria, and acellular pertussis); single dose required for all students age 64 or younger as of January 1, 2012.
  3. Meningococcal vaccine; dose of conjugate vaccine (Menactra, Menveo) required after 16th birthday for all students age 21 or younger as of January 1, 2012.  If received initial dose before 16th birthday, booster is required.  Students must present proof of vaccination, or may sign a waiver declining the vaccination.
  4. Tuberculosis (TB) Screening; required only for international students or residents from high risk countries.

Any student who has resided within the last 5 years in a country that has been identified as "high risk" for tuberculosis is required to be screeened upon arrival to Clemson.  Residents from the following countries are exempt and do not require screning:  Canada, Jamaica, Saint Kitts, Nevis, Saint Lucia, USA, Virgin Islands, Belgium, Denmark, Finland, France, Germany, Greece, Iceland, Ireland, Italy, Liechtenstein, Luxembourg, Malta, Monaco, Netherlands, Norway, San Marino, Sweden, Switzerland, United Kingdom, American Samoa, Australia, New Zealand, Austria.

TB screening must be performed in the U.S. and is valid for one year.

TB screenings performed outside of the U.S. will not be accepted.

Students with a prior positive tuberculin skin test must bring a copy of a chest X-ray report (translated in English), within the last year. Students who are receiving treatment for tuberculosis should bring copies of all treatment records, including medical notes, laboratory reports and X-rays. 

Recommended vaccines

  1. Human Papillomavirus (HPV)
    1. Females 11 to 26 years old: Bivalent or Quadrivalent Human Papillomavirus Vaccine (HPV2 Cervarix or HPV4 Gardasil); series of three vaccines.
    2. Males 9 - 26 years old: Quadrivalent Human Papillomavrius Vaccine (HPV4 Gardasil); series of three vaccines.
  2. Varicella - series of two vaccines.
  3. Hepatitis B - series of three vaccines.  Can also be combined with Hepatitis A.
  4. Hepatitis A - series of two vaccines.

Exemptions

If completion of medical clearance requirements is contraindicated for medical reasons, approval of the healthcare provider is required in Section D of the Immunization Form along with documentation of the need for the exemption.
