How to fill out the Compost Form

The form can be found on the lab web page by going back to the compost section.

                                 

1.       Insert a disk into the A: drive.

2.       Click SAVE to save the executable compost form onto your A: drive – or you can change the target drive if desired.  You now have the form saved where you can reuse it when needed. 

3.       To open the form, go to the drive where you saved it.  Double click on the executable compost form. Enter in the information and print the sheet, but don’t save it.  The blank form will remain on your drive for reuse.

 

The form can be filled out with the computer or you can print off as many blank copies as you need to fill out by hand.

 

If using the computer – use the TAB key to move through the fields.

 

  1. Completely fill in all requested information at the top of the page.

 

  1. Account:  This seven block space is for your account code.  The county Extension office collecting the fees for samples must enter the account code for that county office.  Researchers should enter their personal computer userid.  Other accounts should enter their seven-digit account number.  This is a mandatory fill field if you have an account. If you’re including payment with the sample sent directly to the lab leave this space blank.   

 

  1. Email to account:  If you have a personal account, an email address, and would like to view your reports on the web click or check the little box in the text.

 

  1. Copy to Email:  If you would like someone else to be able to access the report on the web, enter his or her email address in the 30-block field.  If you submit your sample through the county office and include your personal email address here, the county will have web access to your report.

 

  1. Sample Number:  Enter your sample identification number/label in this 8-block space.  Make sure that the sample number on the sample bag matches the sample number on the form for every sample submitted.

 

  1. Click with the mouse type of compost and intended use.

 

  1. Analyses:  Check the appropriate box(es) for the analyses desired (point with the mouse and click to select box).  Write in the fee for that sample.  (The fees will be carried out automatically if using the computer.)

 

  1. Total Amount Due:  Add up the total amount due at the bottom of the sheet.  (The fees will be added automatically if using the computer.)  If paying by check enter your check number.  Make checks payable to Clemson University.

 

  1. Deliver samples, completed record sheets, and payment to the lab or your county Extension Agent’s office for shipment to the lab.  Completed forms must accompany samples to the lab.

 

  1. Hard Copies of Reports:  If you do not have a personal account or have not included an email address in the copy to email space you will receive a hard copy from the County Extension Office located in the county in which you reside.  The county office is also responsible for sending all copy to hard copies if requested.
