Forms 

• Roster Removal -This form is used to have members removed officially from an organization’s roster. President and Advisor signatures required. Turn form into the Fraternity and Sorority Life Office, 602 University Union.
  
• Community Services/Philanthropy - This form should be filled out following any community service or philanthropy activities. There are sections for hands-on community service hours, donations of money (must have back up), and donations of goods (must have back up).
  
• Educational Programming - This form is filled out any time your organization hosts or sponsors an educational program. Programs can be members only or open to the public.
  
• Permission to Initiate - For ALL IFC and CPC organizations, forms are due to the Fraternity and Sorority Life 7 Days prior to Initiation. For NPHC organizations, forms are due 48 hours after you received notification from your National Office.
  
• Hazing Compliance - All chapters must agree to uphold the hazing policies on Clemson’s campus as well as in the state of SC. These forms are due on February 1^st of each year.
  
• Hazing/Academic Release - All potential new members of Greek Organizations must sign this form BEFORE joining an organization.

This page is only accessible to our Chapter Presidents. Presidents will need to use their personal Clemson user id and password to gain access. If you are a president and cannot gain access, please contact the Fraternity & Sorority Life Office.
