Fillable Registration Form Instructions

1. Save the fillable Registration Form to your computer. (Note: If you open it rather than save it, the form will not be fillable.)
2. Enable editing if you get a message about this.
3. Tab through the fields filling them in as you go along.
4. Use "Save as" and rename the file to your agency name. 
5. Either Email it or print it off and mail it.
