What to send to the Department of Pesticide Regulation:     
DO NOT send these to the Department of Entomology, Soils, & Plant Sciences!

  • A completed Private Applicator Certification Exam for each trainee wishing to receive a Private Applicator License.
  • The original Verification of Private Applicator Training sheet.
  • Mail to:

What to Send to the Department of Entomology, Soils, & Plant Sciences, Pesticide Information Program:     
DO NOT send these to DPR!

  • A matched and scored Pretest and a Post-test for each trainee.
  • Copies of your Training Evaluation forms.
  • A copy of the Verification of Private Applicator Training form.
  • Mail Pretest/Post-tests Training Evaluations and Verification Sheet to:

  • Mail or FAX the Printable IDO (Excel file or PDF) for Entomology's portion of the Extension Training fee (which increased to $10 per trainee as of January 1, 2003). Print out the IDO from the web site. Fill in the IDO with your full expenditure account number and the amount of payment and FAX (864-656-0274) or MAIL the IDO to: Tracy Patterson Entomology, Soils, and Plant Sciences Dept. 102 Long Hall Clemson, SC 29634-0315
