Requirements and Recommendations

All entering undergraduate and graduate students are required to submit a verified copy of their immunization record and complete the Tuberculosis Risk Assessment.

Exception: students enrolled in only online courses

To submit a verified copy of your immunization record, either complete page 1 of the Immunization Forms or obtain a verified copy of your immunization record. Acceptable forms of verification include one of the following:

  • Personal shot records that are verified by doctor's stamp or contain a health provider's signature

  • Personal shot records with a clinic or health department stamp

  • Military Records or World Health Organization (WHO) documents

  • Previous college or university records that are verified


Clemson University offers a comprehensive Student Health Insurance Plan. Beginning fall 2017, we will courtesy file as an out-of-network provider for most medical insurance plans and will be adding additional insurance plans throughout the year.

Health insurance is required for the following students who are considered Mandatory Enrollees:

  1. Graduate students enrolled in nine credit hours or more on the main campus  
  2. Graduate students classified as graduate assistants 
  3. International students (graduate and undergraduate)

Medical Insurance 

We accept most insurance cards at our pharmacy. Please complete the Pharmacy Prescription Card Information form and fax or mail it to the pharmacy in Redfern Health Center:
