National Diabetes Prevention Program (NDPP)
The National Diabetes Prevention Program (NDPP) is a free, educational program centered around healthy behavior change for those who are at risk for developing diabetes. The NDPP provides participants with lifestyle coaching, group support and accountability in order to make sustainable lifestyle changes.
The program is a CDC developed program that provides a series of interactive, research-based and focused on nutrition and physical activity lifestyle change. Regularly scheduled group sessions and weekly one-on-one follow ups are provided with a Health Extension agent trained as a Diabetes Lifestyle Coach.

Participants must be:
- Diagnosed with pre-diabetes within the last six months
- 18 years of age or older
- Have a BMI of 25 or greater (23 if of Asian descent)
